Denver Workplace/Organization COVID-19 Outbreak Reporting Form

Denver Department of Public Health & Environment and Denver Public Health are committed to collaborating with local businesses and organizations to investigate and control COVID-19 outbreaks. Please complete the following form to report an outbreak of COVID-19 in a Denver workplace or organization. This survey should take about 10 minutes to complete. If you do not complete the survey with sufficient information (i.e., name of workplace/organization, address of workplace/organization) or do not complete the survey by hitting the 'Submit' button, we will be unable to review the information you provide and follow-up with you and/or you workplace/organization.

Long-term care facilities (Nursing Homes, Assisted Living Facilities, etc.) should send an email directly to DDPHEOutbreak@denvergov.org to report any cases or outbreaks in your facility instead of using this survey.

We will review the information you provide and you or the business will be contacted to discuss next steps. When you finish the survey, please provide your email so you can receive an automatic email upon completion of the survey with additional information. Please also reference the Colorado Department of Public Health and Environment's Workplace Outbreak Guidance to review information about determining outbreaks in a workplace and the typical recommendations and protocols.
